These details set up your hosted Forms Manager portal, subscription account, and initial administrator.

Fields marked Required must be completed.

Forms portal name Required This is the application name that appears on your portal.

Organisation name Required This is the organisation identity shown inside the portal.

Trading or business name optional Used for your subscription invoice when it differs from the organisation name.

ABN, VAT, EIN, or tax number optional Shown on your portal where applicable and retained for billing records.

Primary contact name Required This person manages the subscription and becomes the initial application administrator.

Primary contact email Required Used for subscription management, deployment notices, and the initial admin account.

Step 4

Choose your portal domain.

Every customer gets a dedicated managed portal address. Approved reseller accounts use a neutral hosting address. Business and Business Plus can also attach a custom domain.

Your hosted portal name is required. A custom domain remains optional.

Create these records for your customer domain

For a subdomain such as forms.yourbusiness.com, create the CNAME record pointing to your EleosForms portal.
If you are using an apex domain, use the A and AAAA records instead of a CNAME.
If you want to email forms from your own domain later, add the SPF include shown below to your email sending domain's SPF record.
If you already have an SPF record, update that record rather than creating a second v=spf1 record.
Your DNS hosting provider or domain manager can create these records for you.

Your new portal initially uses the platform-managed sender mailer@eleosforms.com, aligned with its hosted domain. DKIM records for your own sender domain can be generated and added later from inside your Forms Manager portal.
